times the annual
earned income rounded to next higher $1,000.00 (i.e. Annual Salary
$23,200.00
life insurance $48,000.00). Maximum Life Insurance is $150,000.00. Long
term
disability benefits of 66 2.3% of monthly salary to a maximum of
$5,000.00
is applicable after fifteen (15) weeks of incapacitation. Effective
January
1, 1996 premiums for life insurance and accidental death and
dismemberment
are paid by the employer and premiums for long-term disability is the
responsibility
